Not only will our providers treat your existing conditions, we also treat acute illnesses and provide annual wellness exams. We strive to help you improve your quality of life, manage your chronic diseases, and be a partner in your healthcare. We work with other members of the healthcare team, including your specialists and home health care providers like skilled nursing and physical therapists.

Meredith Lewis, APRN, MSN, FNP-BC
Meredith has over 19 years experience in the healthcare field. She established Envision in 2019 and has a passion to help others in their healthcare journey.
Katherine Wollitz, PA-C
Kathy has over 30 years experience in the healthcare field. She has a passion for providing care to the senior population and focusing on quality of care as we age. She is married and has two children.
Chadala Hagans, APRN, MSN, FNP-C
Chandala "Shawn" has over 20 years experience in the healthcare field. She joined Envision in 2024 and brings with her a range of knowledge and years of experience in adult care. She is married and has two children.
Stella Hayes, MD
Dr. Stella Hayes attended Medical School in Reno, Nevada, Family Medicine residency in Jacksonville, FL,
and geriatric Fellowship in Greenville, NC. She is retired from the US Navy after 24 years after retiring in 2017. Dr. Hayes also has personal experience as a care-giver, having cared for her 91 year-old mother at home, and she fully appreciates the value of in-home care. She is married and has three children.
Heather Mashburn
Heather joined Envision Primary Healthcare in 2019 and provides office support. She is married and has three children.
Gail Cosgrove
Gail joined Envision Primary Healthcare in 2020 and provides office support. She is married and has three children and three much loved grandchildren.
